Understanding Accutane and Back Pain
If you're considering Accutane (isotretinoin) treatment for severe acne, or you're currently taking it and experiencing back or other joint pain, you're not alone. Back pain is possible side effects of Accutane treatment, and it's important to understand the relationship between this medication and musculoskeletal symptoms.
The Science Behind Accutane-Related Back Pain
Isotretinoin, commonly known by its brand name Accutane, can affect your musculoskeletal system in several ways. While it is unclear why Accutane can be associated with back pain, it is thought that it may affect the cells that line our joints. This process can make joints and surrounding tissues more susceptible to injury or inflammation from movement or trauma. Another hypothesis is that isotretinoin may increase inflammatory molecules in the joints. However, since some variants of severe acne can also be associated with joint pains, it is also possible these symptoms are related to the underlying acne itself.
Back Pain And Accutane: Are They Related?
Although many Accutane users experience back pain, not all back pain is related to Accutane use. There are two types of back pain Accutane users may experience. Mechanical back pain is back pain caused by musculoskeletal issues, which Accutane does not affect. Accutane can, however, cause inflammatory responses in joints that lead to back pain in some cases.
Mechanical Back Pain
Research indicates that mechanical back pain is the most common type of back pain experienced by Accutane users. This type of back pain is not associated with Accutane use.
In a comprehensive study published in BMC Musculoskeletal Disorders, approximately 62.2% of patients who experienced back pain while on isotretinoin had mechanical pain.
Inflammatory Back Pain
The same study found that about 37.8% of patients with back pain experienced inflammatory back pain. This type of pain typically:
- Improves with exercise
- Worsens with rest
- May cause morning stiffness
- Can disturb sleep, especially in the second half of the night
The Relationship Between Dosage and Back Pain
Musculoskeletal symptoms such as back pain while on isotretinoin appear to be more common in those who are receiving higher daily dosages. Thus, taking a lower daily dose may be helpful to prevent or address these symptoms when they occur.
At Honeydew, we often prescribe Accutane at lower doses to combat side effects like inflammatory joint pain. At higher doses, Accutane may cause more side effects. Recent research shows that Accutane prescribed at lower doses is just as effective as higher dose Accutane when taken for a longer time period but with few to no side effects.
Is This Back Pain Permanent?
In addition to managing pain by adjusting the dose, the good news is that the pain is rarely ever permanent. Most patients who experience joint and back pain see their pain resolve within a few months after they complete their treatment with isotretinoin. This is true both for symptoms and inflammation seen on imaging such as MRI.
Managing Back Pain During Accutane Treatment
If you're experiencing back pain while on Accutane, there are several management strategies that may help:
- Discuss dose adjustment with your healthcare provider
- Consider using NSAIDs (non-steroidal anti-inflammatory drugs) as recommended by your doctor
- Monitor your symptoms and report any changes to your healthcare provider
